Active Ingredient:

• Candesartan cilexetil 8 mg

Therapeutic Class:
Angiotensin II receptor blocker (ARB) – Antihypertensive agent

Indications:

• Treatment of essential hypertension (high blood pressure) in adults and children over 6 years old.

• Management of heart failure with reduced ejection fraction, particularly in patients who are intolerant to ACE inhibitors or when additional therapy is required.

Dosage:

• Hypertension:

  • Usual starting dose: 8 mg once daily.

  • Maintenance dose: may be increased to 16–32 mg once daily based on blood pressure response.

  • Can be used alone or with other antihypertensives (e.g., diuretics, calcium channel blockers).

• Heart failure:

  • Starting dose: 4 mg once daily, gradually increased to 32 mg once daily or maximum tolerated dose.

• May be taken with or without food, preferably at the same time each day.

Mechanism of Action:

• Candesartan selectively blocks the angiotensin II type 1 (AT₁) receptor, preventing vasoconstriction and the secretion of aldosterone.

• This results in reduced blood vessel tension, lower blood pressure, and decreased strain on the heart.

Precautions:

• Contraindicated in:

  • Pregnancy (especially 2nd and 3rd trimesters)

  • Severe hepatic or renal impairment

  • Hypersensitivity to candesartan or any component of the drug

• Use cautiously in patients with:

  • Dehydration, renal artery stenosis, or electrolyte disturbances (especially low sodium)

  • Heart failure or renal impairment—monitor kidney function and potassium regularly

• Avoid using potassium supplements or potassium-sparing diuretics unless prescribed.

• May cause dizziness; rise slowly from sitting or lying positions.

Possible Side Effects:

• Dizziness or lightheadedness

• Headache or fatigue

• Low blood pressure (especially after first doses)

• Elevated potassium levels (hyperkalemia)

• Rare: kidney dysfunction, rash, or angioedema
