Indications:

Edema (fluid retention) due to:
-
Congestive heart failure
-
Chronic kidney disease
-
Liver cirrhosis

Hypertension (high blood pressure) – alone or in combination with other antihypertensive medications
Dosage:
-
Edema: Commonly 10–20 mg once daily; can be increased gradually up to 200 mg/day depending on response.

Hypertension: Usually 5–10 mg once daily.

Administer in the morning to prevent sleep disturbance from nighttime urination.

May be taken with or without food.
Mechanism of Action:
Torsemide blocks the Na⁺/K⁺/2Cl⁻ cotransporter in the thick ascending limb of the loop of Henle, resulting in:

Increased urinary excretion of sodium, chloride, and water

Reduced plasma volume and blood pressure

Relief of edema and fluid overload
Precautions:

Contraindicated in:

Anuria (no urine output)

Severe electrolyte imbalance (e.g., hypokalemia, hyponatremia)

Hepatic coma

Hypersensitivity to torsemide or sulfonamide derivatives

Use with caution in:

Renal impairment

Diabetes mellitus (may elevate blood glucose)

Gout (may raise uric acid)

Patients on digoxin or corticosteroids (risk of hypokalemia)

Regular monitoring of electrolytes, kidney function, and blood pressure is essential.
Possible Side Effects:

Dizziness or postural hypotension

Increased urination and dehydration

Hypokalemia (low potassium)

Hyponatremia (low sodium)

Hyperuricemia or gout attacks

Muscle weakness or cramps
