Nicorandil 5 mg
Therapeutic Class:
Antianginal and vasodilator agent
Indications:
-
Prevention and treatment of angina pectoris (chest pain) caused by reduced blood flow to the heart

Often prescribed for patients with stable angina or when other antianginal drugs (like beta-blockers or calcium channel blockers) are not sufficient or not tolerated
Dosage:
-
Usual adult starting dose: 5 mg twice daily (morning and evening).

The dose may be increased to 10 mg twice daily if needed and tolerated.

Tablets should be swallowed whole with a glass of water, with or without food.
Mechanism of Action:

Nicorandil has a dual mechanism:
-
Nitrate-like effect: dilates coronary arteries and veins, improving oxygen delivery to heart tissue.
-
Potassium channel–opening effect: relaxes vascular smooth muscle and reduces cardiac workload.

Precautions:

Contraindicated in:
-
Severe hypotension (low blood pressure)
-
Cardiogenic shock or acute pulmonary edema
-
Concurrent use with phosphodiesterase-5 inhibitors (e.g., sildenafil, tadalafil) due to risk of severe hypotension

Caution in patients with renal or hepatic impairment.
-
Avoid sudden discontinuation to prevent rebound angina.
-
Regular monitoring of blood pressure is advised.
Possible Side Effects:

Headache (very common at the start of treatment)

Dizziness or flushing
-
Nausea or vomiting
-
Hypotension (low blood pressure)
-
Ulceration in mouth, gastrointestinal tract, or anal area (rare but specific to nicorandil)

Fatigue or weakness
Storage:
Store below 25°C, in a dry, cool place, protected from moisture and direct sunlight. Keep out of reach of children.
