Active Ingredient:

  • Tirzepatide 7.5 mg per 0.6 ml prefilled pen

Therapeutic Class:
Dual GIP and GLP-1 receptor agonist (antidiabetic and weight management agent)

Indications:

  • Type 2 diabetes mellitus in adults to improve glycemic control:

    • As monotherapy when metformin is not tolerated or contraindicated.

    • In combination with other antidiabetic agents such as metformin, SGLT2 inhibitors, or insulin.

  • May also be used off-label under specialist supervision for weight reduction in overweight or obese adults.

Dosage and Administration:

  • Administer once weekly via subcutaneous injection in the abdomen, thigh, or upper arm.

  • May be given at any time of day, with or without food.

  • Rotate injection sites weekly to prevent skin irritation.

  • If a dose is missed, it can be given within 4 days (96 hours); otherwise, skip and take the next dose as scheduled.

  • The 7.5 mg dose is an intermediate step in dose escalation (usual sequence: 2.5 → 5 → 7.5 → 10 → 12.5 → 15 mg).

Mechanism of Action:

  • Tirzepatide acts as a dual agonist at GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 (glucagon-like peptide-1) receptors.

  • Enhances insulin secretion and suppresses glucagon in a glucose-dependent manner.

  • Delays gastric emptying, reduces appetite, and promotes satiety.

  • Results in better glycemic control and significant weight loss.

Precautions:

  • Contraindicated in:

    • Personal or family history of medullary thyroid carcinoma (MTC)

    • Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)

    • Severe gastrointestinal disease

  • Use cautiously in patients with:

    • Pancreatitis history

    • Renal impairment, especially if experiencing dehydration from vomiting or diarrhea

  • Not for type 1 diabetes or diabetic ketoacidosis.

  • Monitor for hypoglycemia when combined with insulin or sulfonylureas.

Possible Side Effects:

  • Nausea, vomiting, diarrhea, constipation (most common, often temporary)

  • Loss of appetite, abdominal discomfort, fatigue

  • Hypoglycemia (mainly when used with other antidiabetics)

  • Rare: Pancreatitis, gallstones, allergic skin reactions

Storage:

  • Store refrigerated (2–8°C).

  • May be kept at room temperature (below 30°C) for up to 21 days.

  • Do not freeze.

  • Keep protected from light and out of reach of children.
