Medication Name: Macrofuran 100mg Capsules

Active Ingredient:
Nitrofurantoin 100 mg (macrocrystalline form)

Pharmacological Class:
Urinary antibacterial (nitrofuran derivative)

Dosage Form:
Oral capsules — 30 capsules per pack

Indications:

• Treatment of acute uncomplicated urinary tract infections (UTIs), especially caused by Escherichia coli, Enterococcus faecalis, and Staphylococcus saprophyticus.

• Prophylaxis of recurrent urinary tract infections.

Mechanism of Action:
Nitrofurantoin is reduced by bacterial enzymes to reactive intermediates that damage bacterial DNA, ribosomal proteins, and metabolic enzymes. This results in inhibition of bacterial growth and cell death.

Dosage and Administration:

• Adults: 100 mg every 12 hours (twice daily) with food or milk.

• Prophylaxis: 50–100 mg once daily at bedtime.

• Capsules should be swallowed whole and taken with meals to reduce gastrointestinal discomfort and improve absorption.

Side Effects:

• Gastrointestinal upset (nausea, vomiting, diarrhea, abdominal pain)

• Headache or dizziness

• Dark yellow or brown urine (harmless)

• Rare: allergic rash, peripheral neuropathy, pulmonary reactions, liver enzyme elevation

Precautions:

• Contraindicated in patients with severe renal impairment (creatinine clearance < 60 mL/min).

• Avoid in late pregnancy (at term) and in infants under 1 month old.

• Use with caution in patients with liver disease or pulmonary conditions.

• Periodic monitoring of renal and hepatic function is advised during long-term use.
