Medication Name: Dolabactin 400 mg Tablets (5 Tablets)

Active Ingredient: Moxifloxacin 400 mg

Pharmacologic Class: Fluoroquinolone antibiotic

Indications:

  • Community-acquired pneumonia.
  • Acute bacterial sinusitis.
  • Acute exacerbation of chronic bronchitis.
  • Uncomplicated and complicated skin and soft tissue infections.
  • Complicated intra-abdominal infections.

Dosage and Administration:

  • Adults: 400 mg once daily.
  • The duration of treatment depends on the type and severity of the infection (e.g., typically 5 days for acute bacterial sinusitis, 7-14 days for complicated skin infections).
  • May be taken with or without food.
  • Swallow the tablet whole with a glass of water.
  • It is crucial to complete the full course of treatment as prescribed, even if symptoms improve early.

Contraindications:

  • Hypersensitivity to moxifloxacin, other fluoroquinolones (e.g., ciprofloxacin, levofloxacin), or any component of the formulation.
  • Patients with a history of tendon disorders or rupture related to fluoroquinolone use.
  • Congenital or documented acquired QT prolongation or patients taking drugs that prolong the QT interval (e.g., Amiodarone, Sotalol).2

  • Uncorrected hypokalemia (low potassium levels).3

  • Children and adolescents under 18 years of age (due to potential risk of joint/cartilage damage).
  • Patients with uncompensated liver impairment.

Precautions:

  • Tendon Rupture/Tendinitis: Discontinue immediately if pain, inflammation, or rupture of a tendon (especially the Achilles tendon) occurs. The risk is higher in the elderly, kidney/heart/lung transplant recipients, and those on corticosteroids.
  • QT Prolongation: Use with extreme caution in patients with cardiac conditions (like bradycardia, ischemia) or those taking other drugs that affect heart rhythm.4

  • Central Nervous System (CNS) Effects: May cause dizziness, seizures, or confusion. Use cautiously in patients with known CNS disorders or epilepsy.
  • Peripheral Neuropathy: Stop the medication immediately if symptoms of peripheral neuropathy (pain, burning, tingling, numbness, or weakness) appear, as it may become irreversible.
  • Photosensitivity: Avoid excessive sunlight and artificial UV light exposure during treatment and for several days after.
  • Drug Interactions: Must be taken at least 4 hours before or 8 hours after products containing magnesium, aluminum, iron, or zinc (including antacids and multivitamins) as these can significantly reduce absorption.5

Common Side Effects:

  • Nausea, vomiting, diarrhea, or abdominal pain.
  • Headache or dizziness.
  • Insomnia (difficulty sleeping).
  • Change in taste sensation.
  • Mild changes in liver enzyme levels.

Serious Side Effects (rare):

  • Tendon rupture (tendinitis).
  • Severe allergic reaction (Anaphylaxis, Angioedema).
  • Serious skin reactions (e.g., Stevens-Johnson syndrome).
  • Life-threatening heart rhythm changes (QT prolongation, Torsades de Pointes).6

  • Severe liver damage or hepatitis.
  • Clostridioides difficile-associated diarrhea (CDAD).

Storage Conditions:

  • Store at room temperature, typically below $25^\circ \text{C}$ or $30^\circ \text{C}$.
  • Protect from light, heat, and moisture.
  • Keep out of reach of children.
