Medication Name: Dapaglif Plus XR 10/1000mg 14 Tablets

Active Ingredients:

• Dapagliflozin 10 mg

• Metformin Hydrochloride 1000 mg (Extended Release)

Pharmacological Class:
Combination of SGLT2 inhibitor and biguanide antidiabetic agent

Indication:
Used in adults with type 2 diabetes mellitus to improve glycemic control when both dapagliflozin and metformin XR are indicated, as an adjunct to diet and exercise.

Dosage:
One tablet once daily, or as prescribed by the physician.
Dose adjustments may be needed based on renal function and blood glucose response.

Administration:

• Take orally with meals to reduce gastrointestinal side effects from metformin.

• Swallow the tablet whole; do not crush or chew.

• Maintain adequate hydration during treatment.

Precautions:

• Evaluate renal function before initiation and monitor periodically.

• Contraindicated in patients with severe renal impairment, metabolic acidosis, or diabetic ketoacidosis.

• Use caution in patients with liver impairment or heart failure.

• Temporarily discontinue before major surgery or administration of iodinated contrast agents.

Possible Side Effects:
Hypoglycemia (especially when used with insulin or sulfonylureas), urinary tract infections, genital infections, increased urination, nausea, vomiting, diarrhea, and abdominal discomfort.
