Active Ingredient:

• Tirzepatide 10 mg per 0.6 ml prefilled injection pen

Therapeutic Class:
Dual GIP and GLP-1 receptor agonist – Antidiabetic and weight management medication

Indications:

• Type 2 diabetes mellitus in adults to improve blood glucose control:

  • As monotherapy when metformin is not tolerated or contraindicated.

  • Or in combination with other oral antidiabetic drugs (e.g., metformin, SGLT2 inhibitors, insulin).

• Sometimes prescribed off-label for weight management under specialist supervision.

Dosage and Administration:

• Inject once weekly via subcutaneous injection in the abdomen, thigh, or upper arm.

• May be administered any time of day, regardless of meals.

• Rotate injection sites each week to prevent skin irritation.

• If a dose is missed, it can be administered within 4 days (96 hours); otherwise, skip and continue with the next scheduled dose.

• The 10 mg dose is typically used after titration from lower doses (2.5 → 5 → 7.5 → 10 mg), before progressing to 12.5 mg or 15 mg if tolerated.

Mechanism of Action:

• Tirzepatide is a dual agonist of GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 (glucagon-like peptide-1) receptors.

• It helps to:

  • Stimulate insulin secretion and reduce glucagon in a glucose-dependent manner

  • Slow gastric emptying

  • Suppress appetite and enhance satiety

  • Result in improved glycemic control and significant weight loss

Precautions:

• Contraindicated in:

  • Personal or family history of medullary thyroid carcinoma (MTC)

  • Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)

  • Severe gastrointestinal disorders

• Caution in:

  • Pancreatitis history

  • Renal impairment (especially with vomiting or dehydration)

• Not indicated for type 1 diabetes or diabetic ketoacidosis.

• Monitor for hypoglycemia when combined with insulin or sulfonylureas.

Possible Side Effects:

• Nausea, vomiting, diarrhea, constipation (common and dose-related)

• Decreased appetite, abdominal discomfort, fatigue

• Hypoglycemia (mainly when used with other antidiabetic drugs)

• Rare: Pancreatitis, gallbladder disease, allergic reactions

Storage:

• Store refrigerated (2–8°C).

• Can be kept at room temperature (below 30°C) for up to 21 days.

• Do not freeze.

• Keep protected from light and out of reach of children.
