Indications:

Edema (fluid retention) caused by:
-
Congestive heart failure
-
Chronic kidney disease
-
Liver cirrhosis
-

Hypertension (high blood pressure) – used alone or with other antihypertensives
Dosage:
-
Edema: Usual starting dose is 10–20 mg once daily; can be increased gradually according to response.

Hypertension: 5–10 mg once daily; may be adjusted if blood pressure remains uncontrolled.

Take preferably in the morning to avoid frequent nighttime urination.

Can be taken with or without food.
Mechanism of Action:
Torsemide inhibits the Na⁺/K⁺/2Cl⁻ cotransporter in the thick ascending limb of the loop of Henle, leading to:

Increased urinary excretion of sodium, chloride, and water

Decreased circulating blood volume
-
Reduction in edema and blood pressure
Precautions:

Contraindicated in:

Anuria (no urine output)
-
Severe electrolyte imbalance (e.g., low sodium or potassium)

Hepatic coma
-
Hypersensitivity to torsemide or sulfonamides

Use with caution in:

Renal impairment
-
Diabetes mellitus (may increase blood glucose)

Gout or hyperuricemia (may raise uric acid)

Patients taking digoxin, corticosteroids, or lithium

Monitor electrolytes, kidney function, and blood pressure regularly.
Possible Side Effects:

Dizziness or lightheadedness

Hypotension (low blood pressure)

Increased urination and dehydration

Hypokalemia (low potassium)

Hyponatremia (low sodium)

Muscle cramps or weakness
-
Elevated uric acid or blood glucose levels
