Active Ingredient:

Captopril 50 mg – Angiotensin-Converting Enzyme (ACE) Inhibitor
Therapeutic Class:
Antihypertensive and cardioprotective agent (ACE inhibitor)
Indications:

Hypertension (high blood pressure)

Heart failure (as part of combination therapy)

Post-myocardial infarction (to improve survival and prevent heart remodeling)

Diabetic nephropathy (kidney protection in diabetic patients)
Dosage:

Usual adult dose:
-
Hypertension: 25–50 mg 2–3 times daily

Heart failure: Initially 6.25–12.5 mg 2–3 times daily, titrated as tolerated

Should be taken 1 hour before meals (food decreases absorption)

Dose adjustment is required in patients with renal impairment
Mechanism of Action:
-
Inhibits conversion of angiotensin I to angiotensin II
-
Reduces vasoconstriction and aldosterone secretion
-
Leads to vasodilation, decreased blood pressure, and reduced cardiac workload
Precautions:

Contraindicated in:

History of angioedema with ACE inhibitors

Pregnancy (especially 2nd and 3rd trimesters)

Severe renal impairment or bilateral renal artery stenosis

Use cautiously in:

Patients with kidney disease or volume depletion

Diabetics using insulin or oral hypoglycemics (risk of hypoglycemia)

Patients on potassium supplements or potassium-sparing diuretics (risk of hyperkalemia)

Regular monitoring of renal function, potassium, and blood pressure is advised
Possible Side Effects:

Dry, persistent cough (common with ACE inhibitors)

Dizziness or hypotension, especially after the first dose

Increased potassium levels (hyperkalemia)

Fatigue or weakness

Rare: Angioedema, rash, or loss of taste (dysgeusia)
