Therapeutic Class:
Class IC Antiarrhythmic Agent
Indications:
-
Treatment and prevention of certain types of serious cardiac arrhythmias, including:
-
Paroxysmal supraventricular tachycardia (PSVT)
-
Atrial fibrillation or atrial flutter (to maintain normal sinus rhythm)
-
Ventricular arrhythmias (ventricular tachycardia or premature ventricular contractions)

Dosage:
-
The usual adult starting dose is 150 mg every 8 hours (three times daily).
-
Dose may be increased gradually up to 300 mg every 8 hours depending on response and tolerance.

Tablets should be swallowed whole with water, with or without food, at evenly spaced intervals.
-
Dosage adjustment may be needed for elderly patients or those with liver or kidney impairment.
Precautions:

Contraindicated in patients with:
-
Significant structural heart disease or severe heart failure
-
Cardiogenic shock
-
Marked bradycardia or severe conduction disorders
-
Known hypersensitivity to propafenone
-

Use with caution in patients with liver or kidney dysfunction, electrolyte imbalance, or asthma (due to mild beta-blocking effect).

Monitor ECG regularly to detect QRS or QT prolongation.

Avoid use with other antiarrhythmic drugs unless strictly supervised.
Possible Side Effects:

Dizziness, fatigue, or headache
-
Nausea, vomiting, metallic taste, or constipation

Palpitations, chest pain, or shortness of breath

Visual disturbances (blurred vision, double vision)
-
Rare: worsening arrhythmia (proarrhythmic effect), hypotension, or heart block
Drug Interactions:
-
May interact with beta-blockers, digoxin, warfarin, or other antiarrhythmics.

Avoid alcohol and limit caffeine as they may increase side effects.
Storage:
Store below 25°C, in a dry place, away from light and moisture. Keep out of reach of children.
