Active Ingredient:

• Metolazone 5 mg

Therapeutic Class:
Thiazide-like diuretic (quinazoline class)

Indications:

• Treatment of hypertension (high blood pressure) when additional diuretic effect is needed.

• Management of edema (fluid retention) associated with congestive heart failure, chronic kidney disease, or when other diuretics are insufficient.

Dosage:

• Initiate under physician’s guidance. A typical starting dose is 2.5 mg once daily; may increase to 5 mg once daily depending on response and condition.

• Take in the morning to reduce risk of nocturia (nighttime urination).

• Follow doctor’s specific instructions; adjust based on kidney function and fluid status.

Precautions:

• Contraindicated in: anuria (no urine production), severe renal impairment, hypersensitivity to metolazone or sulfonamide-derived diuretics.

• Use with caution in patients with electrolyte imbalances (especially low potassium or sodium), gout, diabetes, or in those receiving other antihypertensive or diuretic therapies.

• Monitor: renal function (creatinine, GFR), serum electrolytes (potassium, sodium, magnesium), fluid balance and blood pressure regularly.

• Advise patients to stand up slowly if dizziness occurs.

• Avoid excessive intake of salty foods or abrupt discontinuation without consultation.

Possible Side Effects:

• Increased urination, dizziness, light-headedness (especially when rising from sitting/lying position)

• Dehydration, dry mouth, thirst

• Electrolyte disturbances: hypokalemia (low potassium), hyponatremia (low sodium), hypomagnesemia

• Gout exacerbation (due to increased uric acid)

• Rare: rash, allergic reactions

Storage:
