Epi-Dinitra 5 mg (30 Sublingual Tablets)

Active Ingredient:

• Isosorbide Dinitrate 5 mg – Organic nitrate vasodilator

Therapeutic Class:
Antianginal and vasodilator medication

Indications:

• Relief and prevention of angina pectoris (chest pain due to coronary artery disease)

• Adjunct treatment in heart failure (to reduce cardiac workload)

Dosage and Administration:

• Acute angina attack: Place one tablet under the tongue and allow it to dissolve completely.

• If pain persists after 5–10 minutes, another tablet may be taken.

• Maximum: 3 tablets within 15–30 minutes (seek medical attention if no relief).

• Prophylaxis: One tablet 15–30 minutes before activities likely to provoke angina (e.g., exertion, stress).

• Do not swallow the tablet; sublingual absorption provides rapid onset of action (1–3 minutes).

Mechanism of Action:

• Converts to nitric oxide in vascular smooth muscle, causing vasodilation.

• Reduces preload and afterload, lowering myocardial oxygen demand.

• Improves coronary blood flow to ischemic heart tissue.

Precautions:

• Contraindicated in:

  • Severe hypotension or shock

  • Use of phosphodiesterase-5 inhibitors (e.g., sildenafil, tadalafil, vardenafil) within the last 24–48 hours

  • Severe anemia or increased intracranial pressure

• Use with caution in:

  • Glaucoma, thyroid disease, or hepatic impairment

  • Patients with volume depletion or orthostatic hypotension

• Tolerance may develop with continuous use—intermittent dosing schedules are recommended.

Possible Side Effects:

• Headache (very common, due to vasodilation)

• Flushing or dizziness

• Hypotension (especially when standing)

• Reflex tachycardia

• Nausea or lightheadedness

Storage:

• Store below 25°C in a tightly closed container, protected from light and moisture.

• Keep out of reach of children.
