Synjardy 12.5/1000 mg (60 Tablets)

Active Ingredients:

• Empagliflozin 12.5 mg (SGLT2 inhibitor)

• Metformin hydrochloride 1000 mg (Biguanide)

Therapeutic Class:
Oral antidiabetic combination (SGLT2 inhibitor + Biguanide)

Indications:

• Treatment of type 2 diabetes mellitus in adults.

• Used to improve glycemic control when diet, exercise, and metformin alone are insufficient.

• Helps lower blood glucose by:

  1. Increasing urinary glucose excretion (Empagliflozin).

  2. Reducing hepatic glucose production and improving insulin sensitivity (Metformin).

• May also provide cardiovascular and renal protective benefits in diabetic patients.

Dosage:

• Usual dose: one tablet twice daily, taken with meals to reduce gastrointestinal side effects.

• Dose adjustments depend on previous antidiabetic therapy and kidney function.

• Swallow whole; do not crush or chew.

Precautions:

• Contraindicated in patients with severe renal impairment (eGFR < 45 mL/min/1.73 m²), metabolic acidosis, or diabetic ketoacidosis.

• Monitor renal function before and during therapy.

• Use with caution in elderly patients or those prone to dehydration.

• Temporarily discontinue before major surgery or contrast imaging procedures.

• Avoid excessive alcohol intake to reduce risk of lactic acidosis.

Possible Side Effects:

• Genital yeast infections or urinary tract infections (due to glucosuria)

• Increased urination or thirst

• Nausea, diarrhea, or stomach upset (mostly due to metformin)

• Hypoglycemia (especially if combined with insulin or sulfonylureas)

• Rare: lactic acidosis (a serious but uncommon reaction)
