Therapeutic Class:
Antianginal and vasodilator agent
Indications:
-
Treatment and prevention of angina pectoris (chest pain) due to coronary artery disease

Used in patients who do not respond adequately to, or cannot tolerate, other antianginal medications such as beta-blockers or calcium channel blockers
Dosage:
-
Usual adult dose: 10 mg twice daily (morning and evening).

The dose may be adjusted by the physician, but higher doses are generally not recommended due to risk of side effects.

Tablets should be swallowed whole with water and may be taken with or without food.
Mechanism of Action:
-
Nicorandil acts as both a nitrate donor and potassium channel opener, leading to relaxation of blood vessels, improved blood flow to the heart muscle, and reduced oxygen demand of the heart.
Precautions:

Contraindicated in patients with:
-
Severe hypotension (low blood pressure)
-
Acute pulmonary edema
-
Cardiogenic shock
-
Concurrent use of phosphodiesterase type-5 inhibitors (e.g., sildenafil, tadalafil) due to risk of severe hypotension

Use cautiously in patients with renal impairment or dehydration.

Avoid abrupt discontinuation to prevent rebound angina.
-
Monitor blood pressure regularly during treatment.
Possible Side Effects:

Headache (very common, usually transient)

Dizziness or lightheadedness

Flushing
-
Nausea or vomiting
-
Low blood pressure
-
Mouth or anal ulcers (rare but characteristic adverse effect of nicorandil)

Fatigue or weakness
Storage:
Store below 25°C, in a dry place, away from light and moisture. Keep out of reach of children.
