Icandra Plus 1000/50 mg (30 Tablets)

Active Ingredients:

• Sitagliptin 1000 mg (DPP-4 inhibitor)

• Metformin 50 mg (Biguanide antidiabetic)

Therapeutic Class:
Combination oral antidiabetic – DPP-4 inhibitor + Biguanide

Indications:

• Type 2 diabetes mellitus in adults

• Used to improve glycemic control when diet, exercise, and monotherapy are insufficient.

• May be prescribed for patients who require dual mechanism therapy to manage blood sugar.

Dosage:

• Usual adult dose: One tablet once or twice daily, usually with meals to reduce gastrointestinal side effects.

• Dose adjustments may be necessary based on renal function, blood glucose levels, and tolerance.

Mechanism of Action:

• Sitagliptin: Inhibits DPP-4 enzyme, increasing incretin levels, which stimulate insulin release and decrease glucagon levels in a glucose-dependent manner.

• Metformin: Decreases hepatic glucose production, increases insulin sensitivity, and reduces intestinal glucose absorption.

• The combination provides synergistic glucose-lowering effects through complementary mechanisms.

Precautions:

• Contraindicated in:

  • Type 1 diabetes

  • Diabetic ketoacidosis

  • Severe renal impairment

  • Hepatic disease or metabolic acidosis

  • Hypersensitivity to sitagliptin or metformin

• Use with caution in:

  • Mild to moderate renal impairment

  • Risk of lactic acidosis

  • Elderly patients

• Regular monitoring of blood glucose, renal function, and HbA1c is recommended.

Possible Side Effects:

• Gastrointestinal upset (nausea, vomiting, diarrhea, abdominal discomfort)

• Hypoglycemia (especially if combined with other antidiabetics)

• Headache

• Rare: lactic acidosis, pancreatitis, allergic reactions
