Active Ingredient:

  • Tirzepatide 12.5 mg per 0.6 ml prefilled pen

Therapeutic Class:
Dual GIP and GLP-1 receptor agonist (antidiabetic and weight management agent)

Indications:

  • Type 2 diabetes mellitus in adults to improve blood glucose control:

    • As monotherapy (if metformin is not suitable)

    • Or in combination therapy with other antidiabetic medications such as metformin, SGLT2 inhibitors, or insulin.

  • Sometimes used off-label for weight reduction under medical supervision.

Dosage and Administration:

  • Administer once weekly via subcutaneous injection in the abdomen, thigh, or upper arm.

  • Can be injected any time of day, with or without food.

  • Do not inject into the same site each week—rotate sites to avoid irritation.

  • If a dose is missed, it may be taken within 4 days (96 hours); otherwise, skip and resume the next scheduled dose.

  • The 12.5 mg dose is typically a step-up dose before reaching the maintenance dose of 15 mg, following titration from lower strengths (2.5 → 5 → 7.5 → 10 → 12.5 → 15 mg).

Mechanism of Action:

  • Tirzepatide activates GIP and GLP-1 receptors, leading to:

    • Increased insulin secretion and decreased glucagon levels (glucose-dependent)

    • Delayed gastric emptying

    • Appetite suppression and enhanced satiety

    • Improved glycemic control and weight reduction

Precautions:

  • Contraindicated in:

    • Personal or family history of medullary thyroid carcinoma (MTC)

    • Multiple Endocrine Neoplasia syndrome type 2 (MEN 2)

    • Severe gastrointestinal disorders

  • Caution in patients with:

    • Pancreatitis history

    • Renal impairment (especially if severe vomiting/diarrhea occurs)

  • Not indicated for type 1 diabetes or diabetic ketoacidosis.

  • Monitor blood glucose closely if combined with insulin or sulfonylureas due to risk of hypoglycemia.

Possible Side Effects:

  • Nausea, vomiting, diarrhea, constipation (most frequent, usually transient)

  • Loss of appetite, fatigue, indigestion

  • Hypoglycemia (especially with other glucose-lowering agents)

  • Rare: Pancreatitis, gallbladder issues, injection site reactions

Storage:

  • Store in a refrigerator (2–8°C).

  • Can be kept at room temperature (below 30°C) for up to 21 days.

  • Do not freeze.

  • Keep the pen capped and protected from light.
