Medication Name: Glucophage XR 500mg 30 Tablets

Active Ingredient:
Metformin Hydrochloride 500 mg (Extended Release)

Pharmacological Class:
Biguanide antidiabetic agent

Indication:
Used in adults with type 2 diabetes mellitus to improve glycemic control, as an adjunct to diet and exercise.

Dosage:
One tablet once daily, or as prescribed by the physician.
Dose adjustments may be required based on renal function and therapeutic response.

Administration:

• Take orally with meals to reduce gastrointestinal side effects.

• Swallow the tablet whole; do not crush or chew.

• Maintain adequate hydration during treatment.

Precautions:

• Assess renal function before starting therapy and periodically thereafter.

• Contraindicated in patients with severe renal impairment, metabolic acidosis, or acute conditions that may affect renal function.

• Use with caution in patients with liver impairment.

• Temporarily discontinue before major surgery or use of iodinated contrast agents.

Possible Side Effects:
Gastrointestinal disturbances (nausea, vomiting, diarrhea, abdominal discomfort), vitamin B12 deficiency with long-term use, lactic acidosis (rare), and headache.
